This release form is for anyone under the age of 18 who is planning on flying in the wind tunnel . If the flyer will not have a legal guardian present, this form will need to be notarized.
Any flyers under the age of 18 who do not have a legal guardian present and do not have this form will not be allowed to fly.
This form is a general release form that you can view. You can either download this form now and bring it with you, (unsigned) or fill one out when you arrive at SkyVenture Arizona.
